DWYC – EVENT REQUEST FORM

(Member, Non-member & Port Prepaid Events)

Please provide ALL information requested below and return this form to the Bar Manager at least 2 weeks prior to your requested event date.  We will contact you soon after receipt to confirm the date and that time is available, then to discuss details regarding food and bar requirements, and costs, etc.  NOTE: If the requested date is less than 2 weeks away, please contact Rear Commodore directly at rearcommodore@dwyc.org.    Please PRINT CLEARLY.   Thank you!

If you’d like to schedule a Special Event at the club for a group of more than 10 including members and guests, please download, print and complete this form (please print clearly), then turn in to a member of the Food & Beverage Committee or to the Bar Staff.  F&B will review then contact you to confirm the date and assist with seating and menu needs.  We appreciate your submitting your request form at least 2 weeks prior to the requested event date.  Your Event must be approved by F&B at least 3 days prior to your event date.  While we make every effort to accommodate each request, private events are scheduled on a first-come-first-served basis.  Members are responsible for obtaining confirmation from F&B prior to your event.
