AUTHORIZATION AGREEMENT
FOR AUTOMATIC CREDIT CARD PAYMENTS

I hereby authorize Dana West Yacht Club, hereinafter called “DWYC”, to charge my credit card account on file for amounts due for my club dues and assessments. This authorization does not require DWYC to initiate such charges, and I expressly acknowledge I am responsible for my payments regardless of whether DWYC exercises its authority to charge my credit card and regardless of whether there are sufficient funds available to satisfy the charges. I expressly agree DWYC’s liability under this authorization shall be limited exclusively to amounts which are negligently or intentionally debited by DWYC and which exceed the dues and assessments owed.

I also hereby agree that I am fully responsible for dues and assessments per my membership classification. If paying annually, and I choose to terminate my membership prior to June 30, 2021, I acknowledge that I will be charged the full balance of all outstanding amounts upon my resignation, if any, and no unearned dues will be refundable.

I understand that my credit card will be automatically charged based upon the installment term I check below (yearly or monthly). Thus, it is acknowledged that monthly payments will automatically be charged every month and the yearly payment will  automatically be charged every year until adequate notice to cancel is provided to the club.

THIS AUTHORITY IS TO REMAIN IN EFFECT UNTIL DWYC HAS RECEIVED ADEQUATE NOTICE TO CANCEL WITH A MINUMUM 30-DAY WRITTEN NOTIFICATION FROM ME OF TERMINATION AS PROVIDED BY THE RULES AND REGULATIONS OF DWYC AND WITH SUCH TIME AND MANNER AS TO AFFORD DWYC AND BANK A REASONABLE OPPORTUNITY TO CANCEL.
